Introduction
▶ DEFINITION:
▶ MUMPS is systemic viral Infection caused by
Paramyxovirus [RNA
virus] &
characterized by swelling of the Parotid gland.
●Mumps most commonly
affects Children Between the age
of 5 to 9yrs and it
also affects the Adults.
Mode of
spread
The virus is transmitted by Respiratory Route
▶ 1) by droplet infection
▶ 2) by direct contact
▶ 3) trough saliva and fomites
● Humans are the only natural hosts of the
mumps Virus.
● The peak Infectivity Is 2-3Days before the
onset of
Parotitis & for 3days afterwards.
Epidemiolo
gy
▶The disease occurs throughout the
world.
▶ Although Morbidity rate tends to
be high, Mortality rate is
negligible.
▶ The Annual incidence of Mumps in the
absence of immunization is in range:
▶ Of 100 – 1000 cases/ 100000 population.
CLINICAL FEATURES
➡️. The incubation period ranges from 14-24 days, with a
peak at 17-18 days.
➡️. Approximately 30-40% of infections are subclinical
➡️. In children, prodromal manifestations are rare but may
be manifest by:
* Fever
*Muscular pain (especially in the neck)
* Headache
* Malaise
* Typically precede the parotid swelling by 12 to 24 hours
➡️ Common complaints are
* Earache on the side of parotid involvement
* Discomfort with eating or drinking acidic
food
* Parotid pain is most pronounced during the
first few days of swelling
* Other salivary glands such as the
submandibular and sublingual glands may also
be involved.
* In 10-15% of patients only the submandibular gland(s)
may be swollen
*. Morbilliform rash has been reported in association
with mumps infection
* Systemic symptoms, including fever, usually resolve
within 3 to 5 days
* the parotid swelling subsides within 7 to 10 days
DIAGNOSIS
. A physical examination confirms the
presence of the swollen glands.
. Usually the disease is diagnosed on clinical
grounds
If there is uncertainty about the diagnosis, a
.
test of saliva, or blood may be carried out.
. A newer diagnostic confirmation, using real-
time nested polymerase chain reaction (PCR)
technology, has also been developed.
. As with any inflammation of the salivary
glands, serum amylase is often elevated .
