Urine Benzidine Test
A Diagnostic Tool for Detecting
Blood in Urine
Introduction
• Overview:
• The benzidine test is a qualitative chemical test
for detecting occult blood in biological fluids,
especially urine and stool.
• It is based on the peroxidase-like activity of
hemoglobin, which catalyzes the oxidation of
benzidine.
• Purpose:
• Detects hematuria (blood in urine), especially in
microscopic or occult forms.
Principle of Benzidine Test
• Chemical Reaction:
• Hemoglobin (in presence of H₂O₂) catalyzes the
oxidation of benzidine, producing a blue-green
color.
• Reaction Equation:
Benzidine + H₂O₂ → (Hb as catalyst) →
Blue/Green-colored oxidized product
• Interpretation:
• Appearance of blue-green color within seconds
indicates presence of blood.
Procedure
• Reagents Needed:
• Benzidine reagent (benzidine in glacial acetic
acid)
• Hydrogen peroxide (H₂O₂)
• Steps:
• Mix equal volumes of urine and benzidine
reagent in a test tube.
• Add a few drops of H₂O₂.
• Observe for color change.
Precaution:
• Perform in a well-ventilated area.
• Benzidine is carcinogenic – handle with
PPE and dispose of safely.
Clinical Significance & Limitations
• Significance:
• Useful for screening hematuria in various conditions:
• UTIs, kidney stones, malignancies, glomerulonephritis
• Limitations:
• False positives: myoglobin, oxidizing agents
• False negatives: high ascorbic acid
• Benzidine is no longer preferred due to carcinogenicity
– replaced by safer alternatives (e.g., orthotoluidine,
guaiac test)
