Carpal Tunnel
Syndrome
Carpal tunnel syndrome is
caused by pressure on the
median nerve. The carpal
tunnel is a narrow passageway
surrounded by bones and
ligaments on the palm side of
the hand. When the median
nerve is compressed,
symptoms can include
numbness, tingling, and
weakness in the hand and arm.
Causes
Symptoms:-
These are the most common symptoms:
1- Weakness when gripping objects with
one or both hands
2- Pain or numbness in one or both
hands
3- "Pins and needles" feeling in the
fingers
4- Swollen feeling in the fingers
5-Burning or tingling in the fingers,
especially the
6- thumb and the index and middle
fingers
7- Pain or numbness that is worse
at night, interrupting sleep
Signs:-
1- Pain.
2- Numbness.
3- Tingling.
4- Weaker grip.
5- Tendency to drop things
Risk Factors

sex, age, smoking, wrist injury, diabetes mellitus,


hypothyroidism and wrist working are all risk factors of CTS.
Hypertension could be a protection factor of CTS in early phase
but will increase the risk in a long-term high blood pressure.
Diagnosis

TINEL SIGN ( doctor will lightly tap over the affected


nerve. If the nerve is affected then the patient will feel
tingling that radiates outward. This sensation is called
paraesthesia)
PHALEN TEST ( A patient is instructed to compress the
back of both hands against each other so that wrist
flexes at 90°. If it cause aching or tingling in the
distribution of median nerve within a minute then ths
test is positive)
Diagnosis

CARPAL TUNNEL COMPRESSION TEST ( Apply the pressure with


thumbs over the median nerve within the carpal tunnel, located
just distal to the wrist crease. The test is positive if the patient
repsonds with numbness and tingling within 30 seconds

Management
Surgical Management

During carpal tunnel release, a surgeon makes an incision in the


palm of your hand over the carpal tunnel ligament and cuts through
the ligament to relieve pressure on the median nerve. The surgery
may be done by making one incision on the palm side of the wrist,
or by making several small incisions.
Medication

Nonsteroidal anti-inflammatory drugs (NSAIDs).


NSAIDs , such as ibuprofen (Advil, Motrin IB, others),
may help relieve pain from carpal tunnel syndrome in
the short term.
PT Management

Exercise
️Wrist extension
➡️Wrist Flexion
➡️Median Nerve Glide
➡️Tendon Glide
➡️Wrist lift
➡️Hand Squeeze
➡️Wrist Stretch
Preventio
n
1-Keep your wrists straight.
2-Use a splint or brace that helps keep your wrist in
a neutral position.
3-Avoid flexing and extending your wrists over and
over again.
4-Keep your hands warm.
5-Take breaks whenever you can.
6-Put your hands and wrists in the right position
while you work.
