CHAPTER 4
CHAPTER 4. MONITORING AND EVALUATION
 The School Monitoring and Evaluation Team (SMET) will monitor progress in the
middle and end of project implementation period depending on the timelines set by
the Project Teams. In longer projects, the SMET will have to monitor more than twice.

 Schedules of monitoring must be indicated in the suggested Project Monitoring Report


Form (ESIP Guidebook, page 29). To help inform the SPT of the status of their
projects, the Project Teams will submit their respective progress reports using this
template.

 Attach the Project Monitoring Report Forms of all projects for Year 1 AIP, which
contains the following information: names of project, project objectives and targets,
and the proposed dates of monitoring.

CHAPTER 4. MONITORING AND EVALUATION
The School Monitoring and Evaluation Team (SMET) monitors the
progress of project implementation based on the timelines set by the
project teams. The School Project Team (SPrT) keeps track on the status of
the implemented programs and projects. Schedules of monitoring and
evaluation are presented in the attached Project Monitoring Report Form
and Gantt Chart for Programs and Projects. SMET submits the formative
evaluation every quarter and summative annual evaluation at the end of
the school year to School Planning Team (SPT). A summative outcome
evaluation is submitted to the monitoring and evaluation focal person in
the Division Office after the 3rd year of implementation. The information is

used for the decision making to continuously improve in the delivery of
basic education services, improve the school’s performance to achieve the
desired educational outcomes and to come up for possible adjustments,
actions and intervention to achieve the ultimate goal of this school
improvement plan. The following are the duties and responsibilities of the
