Reference: Walk Security Agency (WSA) Code of Discipline Sections:

 37. Any dishonesty, including conspiracy, theft (or attempted theft), fraud (or
attempted fraud), forgery, or giving false or misleading statements to the
company or employees or persons having dealings with the company.
 5. Giving False Testimony during the investigation.
 10. HOLDING Expired License.
 41. Rendering duty with an expired license.

2. In view of the above-cited reference, you are hereby suspended for fifteen (15) days from your
present post assignment at Mt. Makiling International Oil Inc., Cabuyao, Laguna, effective 17
September 2023 and directed to report to WSA Head Office on 01 October 2023 for your back to
work.

3. Further, during this period, you are not permitted to report to work, contact any of the Security
personnel, engage in any work-related activities, or enter company premises without prior
authorization.

4. Failure to adhere to the terms and conditions of this suspension may result in further disciplinary
action, up to and including termination of your employment.

We take this matter seriously and hope that this suspension will provide an opportunity for reflection
and improvement. We expect your full cooperation during this period.
