INTERVIEW GUIDE
I. Introduction and Discussion of the Informed Consent
Purpose of the research:
Good morning/afternoon, ma'am/sir. We the undersigned Bachelor of Physical
Education student from Eastern Visayas State University Ormoc- Campus are
conducting a research study entitled " Lived Experiences among BPED LET Takers"
This study assesses the impact of MAPEH Licensure Examination to the Bachelor of
Physical Education Graduates of Eastern Visayas State University Ormoc- Campus.
What you will do in this research:
If you decide to volunteer, you will be asked to participate in an interview. You will
be asked about your experiences upon taking the licensure exam before, during and after
taking it, as well as the challenges and opportunities you encountered upon taking the
exam. Follow-up questions can be done. I will audio record the interviews with your
permission so I can make fewer notes. You will not be asked to state your name on the
recording.
Time required: The interview would take 20 minutes to 1 hour.
Risks: Some of the questions may cause discomfort or embarrassment.
Benefits: This is a chance for you to tell us about your lived experiences upon taking
the exam of MAPEH as a Bachelor of Physical Education Graduate.
Compensation: Snack and drinks will be provided.
Confidentiality: Your responses to interview questions will be kept confidential. Your
identity will never be revealed in any instance or time. In the interview, you will be
assigned a random numerical code. Anyone who helps me transcribe responses will
only know you by this code. The recording will be destroyed once my action research is
accepted. The transcript will be kept without your name until the research is complete.
The critical code linking your name with your number will be held in a locked file
cabinet in a locked office, and no one else can access it. It will be destroyed. The data
you give me will be used for the dissertation I am currently writing and may be
employed as the basis for articles or presentations in the future. I won't use your name
or information that would identify you in any publications or presentations.
Participation and withdrawal:
Your participation in this study is voluntary. You have the right not to participate or
leave the study anytime. Deciding to refrain from participating or choosing to leave the
study will not result in any penalty or loss of benefits to which you are entitled. You
may skip any questions during the interview.
Agreement:
(The researcher will ask the volunteer to sign the informed consent/assent in a separate
sheet).

Statement of the Problem:
This study aims to determine the impacts of MAPEH (Music, Arts Physical Education and
Health) licensure examination to the bachelor of physical education graduates. Taking up
the Licensure examination is one big challenge to the Bachelor of Physical education
graduates, in which they have to take up the overall competencies in MAPEH. The study
seeks to assess how well the graduates academic training has prepared them for taking up
the licensure examination.
Specifically, this study aims to address the following questions;
1. How do the bachelor of physical education graduates describe their experiences in
preparing for the MAPEH Licensure Examination?
2. How do the bachelor of physical education describe their experiences during the
examination?
3. What are the challenges experienced by the bachelor of physical education upon taking
their licensure examination?
4. What recommendations can be made based on the results?
