2. This pertains to the CDM Seminar and Drills of newly download 345 NPD personnel at about 12:30
PM February 6, 2026 at NPD Head Quarters

3. I, Patrolman Jayvee O Sacramento,of ,DPHAS NPD hereby submit this written explanation regarding
my absence during the CDM Seminar and Drills for newly downloaded 345 NPD personnel held at NPD
Headquarters on February 6, 2026, at about 12:30 PM.

4. This is regarding my sudden departure from the CDM, I Temporarily left to go to a store to eat, as I
had come from the BMI at 3s Center in Mapulang lupa and had not yet taken my lunch at that time.

I Sincerely Apologize for my Negligence in participating in CDM
