I am Pat. BENZ XAVIER HERNANDEZ CAPONPON, a Police Patrolman with a Badge


Number: 378680 currently assigned at Cavite PMFC unit and detailed at Regional Engineering
Unit 4A as Sheltering PNCO.

I respectfully request a transfer of my assignment to either the Municipality of Mabini located in


the Province of Batangas.

This request is made due to family reasons, as my immediate family resides in the
aforementioned municipality, and my presence is essential for their well-being.

I assure you that this transfer will not hinder my duties and responsibilities as a member of the
Philippine National Police. I am committed to upholding the standards and integrity of our
