A 10 year old boy gives history of stray dog bite. On examination, there are abrasions seen over
his left thigh.

Ans: Local Treatment:


• Gently wash under running water with soap over a period of 15-20 minutes which
reduces chance of getting rabies by up to 80% if washed immediately after dog bite.
• Disinfectants – virucidal agents: Povidone Iodine, Spirit, etc.
• Suturing only if required after 24-48 hours only after RIG.
• Simple, non-occlusive dressing can be done if required
• TT and antibiotics to be given to prevent wound infection

ii. Write about the Intramuscular route of immunization regimen for this case.

Ans: Post-exposure vaccination schedule is based on the injecting 1 ml or 0.5 ml into
the deltoid muscle (or anterolateral thigh in children aged <2 years) of patients
with category II and III exposures.
1. Essen regimen : The 5-dose regimen prescribes 1 dose on
each of days 0, 3, 7, 14, and 28.
2. Zareb regimen : The 4-dose abbreviated multisite regimen
prescribes
2 doses on day 0 (1 in each of the 2
deltoid or thigh sites)followed by 1 dose
on each of days 7 and 21.

1. Clean Water - 1 litre - 5 cupful (each cup about 200 ml.)
2. Sugar - Six level teaspoons (1 teaspoon = 5grams)
3. Salt - Half level teaspoon
4. Stir the mixture till the sugar dissolves

iii. Mention some fluids that should be avoided during diarrhea. [0.5 Marks]
