Q-2, At your site, what are the most significant:
 Occupational Health Hazards?
 Occupational Safety Hazards?
 Wellbeing and mental Health Challenges?

Answer, Word- (598)________________________________________________________________________

1. Occupational Health Hazards

a) Ergonomic hazards while manual material handling:-
One major occupational health risk linked to manual material handling is ergonomic hazards. Material
handling is a key task in our project, involving the manual movement of heavy Electrical Cable Trays,
Laying of Electric Cable from Top of the bridge to inside the girder box, Shifting and Installing of Local
Exhaust Fans (LEV) for a positive ventilation system. To mitigate this risk, we've provided mechanical
aids such as pallet jacks, Cable Drum Jacks, Cable rollers, and chain pulley blocks. These aids are
inspected regularly as per legal and safety requirements, both internally and by third-party certified
inspectors. Ergonomic and manual material handling training sessions are conducted regularly at the
project site to educate workers about potential risks and hazards.

2. Occupational Safety Hazards

a) Electrical Hazards: -
Exposure to high voltage systems during installation of the electrical Substations, Feeder Pillars,
Panels, maintenance in the electrical distribution board, or repair work. Risk of getting electric
shock/electrocution due to improper grounding or insulation of electrical equipment, Potential arc flash
incidents during energizing or de-energizing electrical systems are considered as a significant safety
hazard. Before starting the work, we are obtaining electrical work permits. Some major checkpoints
being ensured are listed here such as a Competent person to carry out the electrical activity,
implementation of the Lockout-Tagout (LOTO) system, grounding provision for equipment, insulating
mats, electrical safety(RCCB) in place, IP-65 Electrical DB& measuring device availability, suitable fire
extinguisher availability, job-specific PPE’s, etc. to execute the task related to electrical work. Training
programs on electrical safety are in our practice to ensure safe execution.

b) Falls from Heights:

Working at elevated heights during installation or maintenance of Compact Sub-Station(CSS), Feeder
Pillers, Installation CCTV Camera, Light fixtures, HVAC Ducts, Firefighting Systems, conduits, cables,
pipes, and thermal insulation poses risks. These risks are managed through a work-at-height permit
system where temporary platforms undergo thorough inspection. For tasks at height, we provide a Man-
lifter(MEWP), MBIU(Mobile Bridge Inspection Unit) light-duty mobile scaffolds capable of holding
personnel and materials. Competent erectors assemble these scaffolds, which are then inspected by
certified inspectors to ensure accurate fixation, fall protection, guardrails, outriggers, caster wheels,
access ladder, and proper scaffold tagging, Inspection Tag. Workers undergo acrophobia tests before
working at heights. Height safety training and daily TBT raise awareness among workers.

c) Falls in Sea Water:

While working over water for the installation of CSS, Feeder Piller installation, Cable laying, Glanding
and termination activity, there was a risk of fall of person in the Seawater, to avoid this we deployed
Mobile Bridge Inspection Unit(MBIU) for smooth entry and exit of manpower, Deployed Speed Boat and
Rescue Team to face an unfortunate emergency. These teams were working 24X7. For these, all crew
members undergo Basic Sea Survival training. The Workmen who were engaged in the project activity
were used to wearing Special PPE Like, Life Jacket, Life West, Lifebuoy Ring, etc. Also, training was
conducted for every person who was working over water.

3. Wellbeing concerns:
The systematic method includes several actions such as health hygiene, pre- and post-employment health
fitness certificates and monitoring by occupational health officer, De-fumigation, health camps, site medical
facilities, and tie-ups with specialized hospitals for emergencies. Also, maintaining proper housekeeping and
conducting cleaning drives are crucial aspects. To effectively tackle these hazards, a comprehensive strategy
involving proper training, risk assessment, implementation of safety protocols, provision of appropriate personal
protective equipment, and fostering a culture of safety and well-being within the organization is necessary.
Regular monitoring and evaluation of safety initiatives are vital for identifying areas.

Answer, Word- (599)________________________________________________________________________

In our project, we prioritize the occupational health and safety(OH&S) of our employees, sub-contractor
workmen and other interested parties. We have established a robust reporting system(HSE monitoring
application) which is an online tool to access the employees from top management to the bottom of the
organization. Dashboards provide senior management with up-to-date information on incidents, near-misses,
and ongoing safety issues.

HSE Online Observation& Audit:

Through this module, HSE non-compliance was logged in the system during the site inspection& audits. The
observations were logged by site HSE personnel in the concerned person's name with the observation
description and suggested corrective action. Immediately auto-trigger mail has been sent to all the key
personnel of the project team to take corrective action against the non-compliance. If the observation is not
closed within a period the escalation mail will be sent to the key personnel concerned, including top
management which starts with the supervisor to the CEO of the organization. The audit findings are In line with
non-conformity, observation, area of concern and opportunity for improvements. The action plan will be
described with the suggestive corrective actions, timeline and responsible key person or interested parties.

Incident Reporting:
Based on the site incidents which may be near-miss or injuries/ dangerous occurrences the relevant details of
the incidents are logged in the system. Once it's logged the auto-generated mail is sent to the project team as
well to the top management. The concern team shall initiate the investigation of the incidents and take
appropriate corrective& preventive action in place and the root cause of the incident and preventive measures
shall be determined and reviewed and approved by the Head of HSE. If the corrective and preventive measures
are not in line with the requirements then this will be rejected and again concerned team members to take
suggested remedial measures. The incident reports also get escalated daily until complying the requirements.

Training& Manhours details:

All the onsite training details must be uploaded in the application with the required details such as training topic,
[Link] participants, training date& hours, etc. HO team will review the training evidence, effectiveness then this
may approved, or it may be rejected due to non-compliance. All the training statistics data will be shown in the
system. In the same way, the manhours are also updated in the system. The site HSE team circulates to all
levels during the monthly review meeting.

External Audits& Compliance Reports

The external audits and regulatory inspections provide an independent assessment of safety performance.
Reports from external auditors and regulatory bodies give senior management an external perspective on how
well the organization complies with legal requirements and industry standards.

a) Compliance Audit Findings: Documentation of any areas of non-compliance or potential risks.

Answer, Word- (590)________________________________________________________________________

In our site, we have implemented a comprehensive system to enable and encourage our staff to report health,
safety, environment(HSE) and well-being concerns/ issues. This initiative is developed to ensure that the
employees can raise their concerns/ issues safer friendly and supportively encouraging a proactive HSE&
wellbeing culture.

1. Systematic Reporting Procedure:

We developed and communicated a systematic clear reporting procedure(SOP) for staff to report any
health, safety, environmental& well-being issues/ concerns. These reporting procedures are easily
accessible via various platforms, and they can understand and adopt the system procedures.

a) Web/phone-based online reporting system:

In the HSE monitoring application users can report their HSE& Wellbeing concerns/ issues through
the online platform. The auto-generated email will be shared with key personnel of the project and
remedial measures will be taken care of immediately.

b) HSE- Suggestion Box

On our site, we have developed and placed the HSE- suggestion box system, which will be placed
at every location at the site where more people gather in a common area. The box will be checked
regularly, all the points will be identified and action will be taken immediately to resolve the issues.

c) Open Communication Reporting system:

Site key personnel were identified& trained to handle any concerns/ issues that have been raised
directly from the staff either one or a group of people. Concerns and issues were raised& addressed
on time. Staff were directly approaching the site HSE team to report their concerns and issues.

2. Training and Awareness for Staff:

We organized various training and awareness programs for the site staff to motivate the importance of
reporting any HSE& well-being concerns or issues.
a) HSE Induction Training – New joining employees received induction training prior to entering
the workplace where they were briefed about all existing hazards, reporting channels, key
personnel and how to report HSE& wellbeing concerns and issues.
b) Employee Refresher training – Based on the training calendar we organized the employee
refresher training on health, safety and wellbeing, we explored the importance of reporting
concerns and issues.

3. Maintaining Confidential& Motivational:

All the reporting data were maintained confidential and gave more importance to the health, safety, and
well-being of the employees. Because of this people feel comfortability& motivated to address any
concerns/ issues with the key personnel.

4. Action Taken& Communicated:

Based on the reports from staff, the committee addresses the issues& takes immediate action on health,
safety and well-being issues. And the same to communicate to the staff about what kind of control measures
we take and the status& outcome of the issue.
Here we described how communication has been passed to staff about addressed issues.
a) Daily Toolbox Talk- During the daily toolbox talk the reported issues are communicated among the
staff& what action we took in place and what the remedial measures/ status and outcomes, issues
were addressed or not.
b) HSE Noticeboard- displayed on the noticeboard about the status of the issues and corrective
actions that took place.
c) HSE Committee Meeting- During the HSE committee meeting the issue status and remedial
implementation part shall be discussed and communicated to the staff.

assessment and employer& regulatory requirements. This schedule is reviewed annually, and
necessary updates are made to changes in operation or new OH&S guidelines.
2. Conducting Audit: The audit covers site inspection, document review, employee interaction, and
identifying critical issues, incident summary, training, inspection of potential activities performed. Which
ensures that comprehensive statement of our OH&S performance.
3. Audit report and action plan: Upon the audit the reports are generated and communicated to all the
key personnel. The audit findings are In line with non-conformity, observation, area of concern, and
opportunity for improvements. The action plan will be described with the suggestive corrective actions,
timeline, and responsible key persons or interested parties.
4. Follow-up Audit/ Compliance report: Upon the audit report being circulated, the key person can
implement the suggested corrective actions at the site and the audit compliance report shall be
submitted to the audit team for review and verification. Also, follow-up audits will be scheduled to
validate that the audit findings are compiled.

Example of Audit Improvement:

1. OH&S Poster and Signages- Upon a recent audit by the Employer- it was noticed that the safety
poster and signages were displayed not sufficiently and some of the posters were found damaged
condition. We have established inspection schedules& checklists for better tracking& checking the
poster's condition. We have replaced the damaged posters, and additional posters were displayed in all
areas. This is inspected and monitored weekly. Findings will help us to improve our site safety
awareness about the OHS system among the employees.

We implemented a robust system for identifying& reporting workplace hazards and near-miss cases. This
system is designed and developed to ensure that all Health, safety, and environmental concerns are reported,
and corrective action is taken promptly. This will help us to minimize the potential risk involved in the workplace&
improve the Health Safety& Environment(HSE) standards continuously.

HSE Monitoring Application- The digital platform tools were used across all the project sites in our
organization. This tool will help us to report hazards, near misses, and incidents on an immediate real-time
basis. This can be easily accessible by the employees and top management. All the observations and other
incident analysis were reviewed during the monthly HSE meeting.

Hazard Identification& Reporting System:

a) Daily HSE inspections:
 Daily safety checks have been carried out by the site HSE team along with the execution
team before starting the activity. This inspection prioritizes the identifying hazards at the
workplace.

b) Risk Assessment:
 Risk assessment conducted regularly for all activities and areas that pose potential
hazards. The assessment will evaluate the likelihood and determine the control measures
to minimize the risk.

c) Reporting:
 Any identified hazards are logged in the HSE monitoring application and an auto-generated
email notification is sent to all the team members. Corrective action shall be taken by
concerned personnel, and compliance will be reviewed and approved by the site HSE
personnel.

Near Miss Reporting System:

a) Near Miss Reporting channels:
 The site team can report near misses and incidents by using the online portal, through
phone calls, or by approaching the site HSE team and supervisors.

b) Investigation and follow-up:

 Once a near miss is reported, investigation is carried out by investigation committee
immediately. This involves identifying the root cause and implementing control measures to
prevent future occurrences.

c) Awareness Training:
 The near-miss report is shared with all the team members during the safety committee
meeting and daily TBT. This provides opportunities for improvements and lessons learned
to ensure a safe workplace.

Example of Hazards: Work at Height Hazard:

i. Observation: During the routine site inspection, observed that the working platform was unsecured in
the mobile scaffold. This poses the fall from height hazard and the working personnel may fall from
height due to insecure working platforms.

ii. Corrective Action: The hazard was immediately reported by using the HSE monitoring application to
the team members. Work has been suspended, the scaffolding team deployed at the site to fix the
working platform properly& re-inspected by a competent person. The relevant checklists, inspection
tags and permits were validated and approved to perform the activity.
BRITISH SAFETY COUNCIL
INTERNATIONAL SAFETY AWARD- 2025
MTHL- Rev: 01
iii. Outcome: The result, we have initiated the frequent inspection of Scaffolds and elevated working
platforms before performing any activity to avoid such unsafe conditions in future.

Example of Near miss:

i. Incident Description: Workers were engaged in duct installation at an elevated height, utilizing a
mobile scaffold. During the task, while hammering the duct, the head of the hammer detached from the
handle and fell, and near-miss occurred.

ii. Corrective Action: Near-miss was reported through the HSE Monitoring application and phone call to
the HSE and in-charges, immediately investigation was carried out to identify the root cause of the near-
miss& worker reported that the hammer's head was already loose. The corrective and preventive
measures are in place that provide on-site job-specific and behavioral training and frequent inspection
of all the hand tools. Additionally, non-slip hand tools were implemented.

iii. Outcome: This near-miss incident led to an increase in the frequency of hand tools checks and
ensuring non-slip hand tools. This will ensure similar near-miss cases are prevented.
