Course Outcomes:

Identify the basic structure and function of the human body systems, how the
interact with each other and changes associated with aging as appropriate to a
Personal Support Worker.
Identify common diseases and disorders, causes, sign and symptoms, medication
management of the human body systems throughout the disease trajectory
related to activities of daily living.
Connect knowledge of diseases and disorders of the human body systems with
the support workers role to provide non-pharmacological and client-centered
approach to care as directed by the plan of care in accordance with an
organization policies and procedures.

Instructions for Assignment:

1. The template below must be used as your working document and the document
you submit for marks in word or PDF format.
2. All answers should be in the group’s own words and not copied and pasted from
a source unless indicated. Remember it must answer the question, and you need
to be able to explain your answers.
3. All answers must have a source included or will not be included in your mark for
assignment. For websites copy and paste the link used.
4. Source must be from textbook or evidence informed source. Can not include
class module lectures (PowerPoints) as a reference. Refer to evidence informed
source information in course shell for support or connect with your instructor.

Consider the research that you have done and group learning about the disorder.
Create a case study about a client that has this disorder that includes the information in
your research above. Provide enough information so that your class will be able to
understand your client and the information in the care plan. The care plan should be 1-2
paragraphs, not longer. You at minimum need to include the following in your case study
but may have more details included:
 Client demographics (name, age, gender and any other important facts related to
your disorder)
 Setting (home, LTC, Retirement)
 History of disorder (when diagnosed, any other co-morbidities)
 Impact of disease on client
 Information about ADL and IADL support required
 Team Members involved in care including IP team members and family

24. Care Plan

Use the Care Plan Template at the end of this document to complete a care plan for
your client based on your case study. You are not allowed to use generative AI for
this part of assignment. Include the specific tasks and roles that would be included in
your role as a PSW. Other tasks on care plan that are outside of your scope of practice
indicate the IP team member that would complete. For example: Physiotherapy
Sessions 3 times a week with Physiotherapist. Remember the care plan needs to reflect
your case study and what you have learned about the clients needs in your research
above.
25. Case Study update

Consider the research that you have completed and what you have learned about the
