Subject: Request for permission regarding irregular attendance due to


medical condition

Respected Sir,

I, Tawish Alam, father of Abid Ali (Class 12th, Section A, Roll No. 51),
respectfully wish to inform you that my son has been suffering from a chronic
medical condition since early childhood. He lost his right kidney at the age of
two due to a severe infection. His remaining left kidney also shows signs of
swelling and scarring as per medical reports.

Due to this condition, he often experiences fluctuations in blood pressure,


fatigue, and weakness. Occasionally, he needs hospital visits or rest at home.
As advised by doctors, he is required to avoid excessive physical and mental
stress.

Considering his sensitive health, we kindly request you to allow him to attend
school only 1–2 days a week, depending on his condition. He is committed to
staying updated with all classwork and assignments and will ensure his
presence during all internal assessments and examinations.
