Subject: Request for Leave of Absence for 3 months due to Tennis Elbow

Respected Sir/Madam,

I am writing to formally request a leave of absence from school for a period of three
months, I Varsha Patil parent of Nishika Patil studying in 10 lotus roll no 23.

I have been recently diagnosed with severe Tennis Elbow (Lateral Epicondylitis),
an overuse injury that causes significant pain on the outside of my elbow due
to small tears in the tendons that attach my forearm muscles to the bone. Her
attending physician, Dr. Shiv Gandhi has advised complete rest for my affected arm
to allow the tendons to heal properly. The doctor's recommendation of three months
of complete rest is to ensure a full recovery so that she physically able to write my
board examinations when they commence.

Due to the nature of this injury and the required treatment protocol, she unable to
attend regular classes or perform the necessary academic activities that require the
use of my arm (e.g., writing notes, completion,).

I assure you that she will take full responsibility for catching up on all missed
academic work and syllabus upon my return. She will coordinate with my teachers
and classmates to ensure minimal disruption to my studies.

I have attached the official medical certificate and relevant reports from the
orthopaedist for your verification.

I kindly request your understanding and approval of leave application to facilitate my


recovery and enable me to appear for the board examinations successfully.
