APPLICATION FOR INDUSTRIAL ATTACHMENT (01 January 2025- 31 December 2025)

The Polytechnic is hereby applying for industrial attachment with your organization for , ID who is currently
studying for a National Certificate in Fabrication Engineering. The student completed first year of study during
which the following subjects were covered:

The student is required to be on attachment from 01 January 2025- 31 December 2025. During the attachment period
the student’s supervisor will liaise with you regularly with regard to the student’s conduct and attitude towards work.
