Form I – Inspection Book

Form II – IA Certificate During Construction

Form III – Constructor’s Certificate of Manufacturer and Test

Form IV – Steel Maker’s / Material Certificate
Purpose: Foundation of material traceability; proves material meets Reg 13.
Issued by: Steel mill / material supplier (well‑known manufacturer).
Validity: Per heat / batch.

Forms III‑A to III‑F – Component Certificates
Purpose: Certificates for individual components (pipes, tubes, fittings, mountings etc.).
Issued by: Respective component manufacturers.

Key point: Support Form II by proving each pressure part is IBR‑compliant. [1]
Form V – Provisional Order
Purpose: Temporary permission to use boiler before final registration.
Issued by: Chief Inspector / Director of Boilers.
Validity: About 3–4 weeks only.

Form VI – Certificate for Use of Boiler
Purpose: Main operating licence for registered boiler.
