student of the College of Allied Medical Sciences (CAMS), willingly express my support for the
WU-P CAMS Team Dance who will represent PHISMETS Central Luzon in the upcoming
PHISMETS National Congress to be held on October 22, 2025, at Manila Central University,
Caloocan City.

I hereby agree to contribute the amount of ₱50.00 (Fifty Pesos Only) as part of the team’s support
fund. This contribution will be used to assist the team in their preparation and participation for the
said national event.

I fully understand that this contribution is voluntary and exclusive to BSMT students of Wesleyan
University – Philippines, serving as a gesture of unity and support for our college representatives.
