1. OBJECTIVE

2. GENERALITIES
The work permit is a written document authorizing the performance of a specific job under certain conditions.

3.1. Use of the work permit


The use of work permits is mandatory in the following cases:
Open flame work
Work at height
Heavy or complicated handling work
Work in restricted spaces, confined spaces, and in storage tanks and vessels.
Work on pressure devices and capacities and on HP (High Pressure) circuits
Work in the presence of gas
Modification or blockage work on passages
Work on electrical circuits
Work with radioactive sources
Work with or in the presence of explosives
Excavation work
Non-routine work.

3.2. Precautions to take before starting a job


The site manager or the SSE manager must ensure that the work permit clearly states:
- The work to be done, the procedure and the specific tools for carrying out the various operations (a single location,
a single piece of equipment;

- All safety conditions to be taken, and that all stakeholders:


. Have the required qualification;
. Have a perfect knowledge of the risks, the site, the installations on or near which
they work;
. They understood and accept the task to be executed;
. Knowing to whom they have to answer;
. Knowing that they must close the work permit.
The work permit must be co-signed by the applicant and/or the executor before the start and at the end of
operations.
The HSE manager and/or the site manager are responsible for defining the measures and precautions.

The lead electrician or the electrician on duty is responsible for the electrical insulation of motors and
electric circuits with padlocks or other comparable means.
The chief mechanic or service mechanic is responsible for the mechanical insulation of the parts.

of equipment seat interventions.
The chief electrician or the electrician and the chief mechanic or the mechanic are also responsible for

the equipment in question.

The Security officer and/or the site manager must also:


Ensure that the staff working on the site have received the appropriate safety training;
Ensure the rational use of personal and collective protective equipment by this personnel;
Ensure the control of work areas;
Organize a safety meeting and, if necessary, a safety drill before starting the work.

3.3. Work Permit Application Form


Use the company's 'WORK PERMIT' forms.
The form must be correctly filled out and signed in three copies.
It must be completed at the beginning of each post until the work is finished.
The work permit must be signed by the representative of the project manager.
One must be submitted to the office of the M.O representative when the work is completed.
The other copy will be kept by the person who performed the work.
The original will be kept by the site manager and filed in a special register.
