What is a fishbone diagram?
FISH?
Also known as the Ishikawa diagram
The method consists of clearly outlining what the problem has been and
carry out a brainstorming session by all members to
analyze one by one all the possible root causes of that problem. To
apply it, we can not only make a better decision regarding the
resolution of the problem, but we can also correct errors or
avoid parallel trials fueled by prejudice.

potential incidents, in which the analysis can also present
complexity and it is unknown a priori what the causes may be
main.

HOW TO CREATE A FISHBONE DIAGRAM?
1. Create a header that lists the problem or topic to be studied.
2. Create a backbone for the fish (a straight line leading to the head).
3. Identify at least four 'causes' that contribute to the problem. Connect these four.
causes with arrows to the spine. This will create the shape of a fishbone.
4. Brainstorm on each 'cause' to document things that
they contributed to the cause. You can use the 5 whys or another analysis process of the
root cause to keep the conversation focused.
5. Continue breaking down each cause until all the causes have been identified.
