General Instructions:
1. Individual activity: Kanya-kanya syndrome na tayo. Papel mo, sagot mo!
2. Prepare a cover page with your name, subject/grade level and section, teacher, and
date. You can use folder, long bond paper, or intermediate paper for cover
page. Pwede lagyan ng design (border or corner design) ‘wag lang mag lagay ng mga
tambo, bubog at plastic tulad niya.
3. Use yellow paper/intermediate paper sa pagsagot ng activity. You can use
coloring materials for designing your paper.
4. Write the question and activity name before the answer.
DAY 1: (10 points)
Activity 1: True or False. Copy and answer.

Activity 4:
Performance Task (50 points)
1. Cut out 12 pictures of yourself, starting from when you were a few months old up to the
present.
2. Paste the pictures on yellow paper in chronological order.
3. Label each picture with your age at the time it was taken.
4. This will allow us to see the development and changes over time, similar to the concept of
evolution.
