Subject: Submission of OPD Prescription for Absence.

Respected,

This is to bring to your kind notice that my son, Puneet Sharma (Roll No.
0824015), was unable to attend college due to medical reasons.

Period of Absence: 15th September 2025 to 27th September 2025.

The absence was necessitated by a diagnosis of aphthous ulcer on uvula and


anterior pillars. He has been under the care of Dr. Digvijay Singh Rawat, ENT, at
his place of residence. He is now fit to resume classes from 29th September 2025.

I, Mahesh Chand Sharma, father of Puneet Sharma, therefore request you to kindly
consider the attached OPD prescription issued by both Dr. Sandeep Trehan and Dr.
Digvijay Singh Rawat and grant approval for his absence during the above-
mentioned period.
