INSTRUCTIONS
1. Each job or operation consists of an ordered sequence of tasks. Ensure that all tasks required to perform the job have been listed. Use another sheet if you run out of space.
2. When identifying hazards, all logical possibilities that can cause harm or illness must be considered. When evaluating each task, ask yourself: "WHAT COULD CAUSE AN ACCIDENT IN THIS TASK?"
3. Consider the risks when evaluating each step of the work, what you cannot see but can predict might happen, such as: falls from different levels, contact with electricity, explosion, etc.
4. The consequence, which is the severity of what could happen, such as: traumatic brain injury, spinal cord injury, contusions, FATALITY; electric shock, respiratory arrest, burns, deafness, etc.
5. Write exactly what you need to do to keep each risk under control, such as: using a safety harness in perfect condition with an energy-absorbing lanyard and life line, marking and delimiting the area, verifying the use of the appropriate tools for the task, having personnel to
experience and trained for the execution of this work, use of special protective equipment, etc. Avoid general comments such as 'BE CAREFUL'.
