I, Koushlya Devi W/o Roop Chand Resident of village Thalora, Tehsil Majalta, Distt.
Udhampur (J&K) do hereby solemnly affirm and declare as under:

1. That I am a permanent resident of J&K Union Territory (UT).


2. That I am owner/Co/share/lesse/Tenant/Attorney of land falling under
Khasra No………………………………. and …………… kanals situated at village
Thalora Tehsil Majalta Distt Udhampur.
3. That I am applying for Subsidy/Department benefit for Flowers from
floriculture department Jammu.
4. That I am unemployed or not an employee of semi-Govt./Central Govt.
5. That I have not availed any benefit on this unit/crop from any other
department for the current years.
6. That I shall abide by all the rules and regulation under centrally sponsored
scheme like MIDH, RKVY and Capex Budget.
7. That it is my true statement.
