Annotation for Indicator 1.2.

Indicator: Used research-based knowledge and principles of teaching and learning to

enhance professional practice
MOVs Presented: Daily Lesson Plan, Activity Sheets, Assessment Materials, COT
Rating, and Notes

Annotation

The teacher consistently applied research-based principles of teaching and

learning in planning, implementing, and assessing instruction. The Daily Lesson Plans
reflect the integration of constructivist and learner-centered approaches that promote
active engagement, critical thinking, and collaboration among pupils.

Activity sheets and assessment materials were designed to cater to diverse

learning styles and abilities, following the principle of differentiated instruction to
ensure mastery of competencies. Strategies such as scaffolding, formative
assessment, and contextualization were applied, aligning with current educational
research emphasizing the importance of meaningful learning experiences.

The COT rating and classroom observation notes validate the effective
application of these principles, showing continuous improvement in instructional delivery
and learner engagement. These demonstrate the teacher’s commitment to enhancing
professional practice through reflective teaching informed by research-based
strategies.
Annotation for Indicator 5.5.2

Indicator: Utilized assessment data to inform the modification of teaching and learning
practices and programs
MOVs Presented: Test Papers with TOS, Quarterly Test Results, Least Mastered
Skills, Remediation Plan, Accomplishment Report, and Activity Sheets

Annotation

Assessment data from quarterly tests, supported by the Table of Specifications

(TOS), were systematically analyzed to identify least mastered skills among learners.
The results served as the foundation for modifying teaching practices and designing
targeted remediation plans that addressed specific learning gaps.

The teacher used these data to adjust lesson pacing, integrate differentiated
instruction, and provide additional activity sheets and performance-based tasks
tailored to learners’ needs. Remediation sessions and enrichment activities were
implemented to reinforce difficult competencies and strengthen mastery.

The accomplishment reports and test results demonstrated significant

improvement in learners’ performance after the interventions, reflecting the
effectiveness of using data to guide instructional decisions. This process highlights the
teacher’s commitment to evidence-based teaching and continuous improvement of
classroom practices and learning programs.
Annotation for Indicator 6.1.2

Indicator: Maintained learning environments that are responsive to community contexts

MOVs Presented: Communication Letter/Notice of Meeting, Narrative Report

Annotation

The teacher maintained a learning environment that was responsive to the

needs, values, and participation of the community by fostering strong collaboration
with parents and stakeholders. Communication letters and notices of meetings were
sent to inform and involve parents in school activities and learning support initiatives,
ensuring alignment between home and school efforts.

Through consultative meetings and community-based engagements, the teacher

gathered feedback and insights that helped adapt classroom activities and learning
programs to suit learners’ real-life contexts and community resources. The narrative
report reflects how these collaborations led to improved learner participation, project
implementation, and a more inclusive and supportive classroom environment.

This practice demonstrates the teacher’s commitment to building a learning

atmosphere that extends beyond the classroom, ensuring that teaching remains
relevant, culturally sensitive, and anchored in community partnerships.

school-based and division-led trainings, webinars, and LAC (Learning Action Cell)
sessions, the teacher gained updated pedagogical strategies, curriculum insights, and assessment
practices aligned with current educational research and DepEd priorities.

Participation in these professional learning opportunities fostered collaboration and

knowledge sharing among colleagues, leading to improved lesson planning, assessment design,
and integration of ICT tools in instruction. The certificates of training, LAC session

Reflective Notes on Trainings Attended and Application to Teaching Practice

Teacher: Junn Arnie A. Mercado

School Year: 2023–2024
Subject/Grade Level: Science – Grade 4

Reflection:

Throughout the school year, I actively participated in several professional

development trainings and LAC sessions aimed at enhancing my teaching
knowledge and skills. These opportunities provided valuable insights into innovative
strategies, assessment practices, and the integration of technology in the classroom.

One of the most impactful learnings from the trainings was the importance of
learner-centered instruction and the use of differentiated strategies to address
diverse learning needs. I applied these principles by designing interactive lessons,
engaging activities, and remediation plans tailored to my pupils’ performance levels.
By integrating ICT tools, educational games, and visual aids, I was able to make
lessons more meaningful and enjoyable, particularly in Mathematics concepts.

The sessions on assessment and data utilization strengthened my ability to

analyze test results and identify least mastered skills, which guided the preparation of
intervention materials and activity sheets. This approach helped improve students’
academic performance and confidence, as reflected in their post-test results and active
participation in class.

In addition, collaboration during LAC sessions fostered a sense of professional

community. Sharing best practices and classroom innovations with fellow teachers
allowed me to refine my techniques and adopt new methods that promote critical
thinking and problem-solving among learners.

Through these experiences, I realized that professional growth is a continuous

process that requires reflection, openness, and commitment to improvement. Applying
what I have learned from trainings has not only enhanced my teaching effectiveness but
also inspired me to become a more innovative, reflective, and responsive educator.
