All unsafe practices and incidents will be investigated to determine
what happened and why. All necessary steps will be taken to
prevent a recurrence.

rules. Management at all levels has responsibility for
implementing and maintaining the standards and rules

OPERATIONAL RISK MANAGEMENT (ORM)
WHAT IS ORM?
• Is a decision-making tool that helps us to manage risk.
ORM PURPOSE
• To have safe, stable and sustainable production
4 LAYERS OF ORM PROCESS
• Risk is managed at different levels in the organisation
• We use different Risk Management tools to manage all aspects of risk in our
operations
• We identify the highest risks by reviewing all hazards associated with all aspects of
our operations
• We develop controls to prevent unwanted events from occurring, and controls that
minimise the impact if they do occur
• We also have controls that assist us with recovery from unwanted events
• All high-risk tasks are implemented with controls that are verified

ORM CONCEPTS AND TERMINOLOGY
Hazard A source of potential harm
Risk The chance of something happening that will have an impact on business objectives
Unwanted event/incident A potential situation or condition where the hazard is released
Priority Unwanted event
Control Is part of risk management involving the selection of appropriate preventive and corrective measures, guided
by the Hierarchy of Controls
Critical Control Those controls that significantly influence the likelihood and/or consequence of an event and if removed they
will significantly influence the risk rating.
Work Execution Document Is a summary of the relevant JRA used together with SLAM during task execution
(WED)
SLAM
High Potential Hazard (HPH) Is a condition or situation which can materialize to a consequence rating of level 4 or 5 due to absence of
controls

A control is a barrier which provides means to
reduce the likelihood of an unwanted event
occurring and/or minimize the consequences once
an event has occurred
A critical control is one that is crucial to preventing
an unwanted event or to mitigating the
consequences if it does happen. These significantly
influence the likelihood and/or consequence of an
event and if removed.

WORK EXECUTION DOCUMENT (WED)
• The WED is a summary of the Job Risk Assessment (JRA ) which was
developed in ORM Layer 3 Task RM
• The WED consists of
– Task steps
– Controls to be used during execution
– Verification questions related to Controls
• The WED is completed as the task progresses, step by step
• If a control is not present, not functioning or does not comply with the
requirement – it must be marked as “Not Compliant (NC)”, even if you
were able to correct it along the way
• Remember to capture feedback to help improve the process.
