AFFIDAVIT

I, Anuradha Dev, W/O Anup Dev by profession- Contractor,


resident of Vill- Dakhin Golaghatia Basti, PO- Golaghatia Basti,
PS- Hojai, Dist- Hojai, Assam, do hereby solemnly affirm and
state as follows-
1. That, I am an Indian Citizen by birth and permanently residing
in the above noted address. This is true.
2. That, I am a Class- II contractor under PHE, Hojai. This is true.
3. That, the following machineries/equipments are owned on
lease and hired and possesses by me –
