Report Card
Writing Manual
General Notes
• Write the child’s full name, FIRST and LAST name, on the front and the LEVEL that they are currently
registered in.
• On the back of the Report Card include:
Instructor Name: Your FIRST NAME (do not write your last name)
Date: Session and year e.g. “Fall 2019”
Next Level to Register in: Fill in the appropriate level (One level higher if the child
passes, the same level if the child fails)
• Your worksheets MUST match your report cards
• If you are passing a swimmer, they MUST be passing EVERY item to standard!
• Highlight the failed items and check off (✓) the passed items
• Make sure you comment on AT LEAST TWO items, whether the swimmer is failing or passing. Report
cards for both passing and failing swimmers must include constructive comments. Pick the comments
that pertain to the individual swimmer
• Hand in completed worksheets with your report cards on the due date to your deck supervisors. Refer
to dates to remember.
• If a child does not pick up their report cards by the last class place it in the pickup file at front desk by
the swimmers last name.
• If you think they are close to passing an item, but they are not up to the standard yet, you must still
incomplete (F) it on the worksheet and the report card. It can always be re-evaluated the following week
if they end up passing that item.
• Write using only the “Must Sees”
• Fill up all the lines for the commented section when writing.
DO NOT LEAVE ANY SECTION OF THE REPORT CARD BLANK, fill in the all sections of the report
card including next level they should register in, your name and session date.
If there is any problem with this let the DS know. If you do not meet the standard expected for report
cards, you will be asked to redo them.
This report card manual was created to assist staff in writing report cards which will allow students to
better understand where they still need to improve in their swimming lessons. Please use the pre-
made phrases in your report cards in the charts below. Comments are sorted by level, skill, and even
elements of the skill where children struggle the most.
Note: If you want a second opinion on a particular skill, leave the checklist blank and ask the
Deck Supervisor to re-evaluate. Still comment on this particular skill and outline what they have
missed as per the template.

First Aid
Assess conscious victim Remember to check the environment for hazards, your victim’s
responsiveness, and any potential areas of bleeding. Also, be sure to
communicate with your victim to gather relevant information.

Care for bleeding Remember to check the environment for hazards, establish responsiveness,
and bleeding. Ensure you check ABCs, rest and reassure your victim, and
apply direct pressure to the injury.
Victim Remember that weak swimmers might have some swimming abilities and are
simulation/recognition: usually at the surface of the water, but have difficulty reaching safety. They

your victim in a comfortable position and assist your victim to find and use
their medication.
Rescue with towing aid Remember to call for help before entering the water. Make sure to keep your
eyes on the victim by entering the water with a stride jump and swimming to
the victim using a head up approach. Remember to always communicate with
your victim throughout the rescue by providing reassurance and appropriate
care.

Drowning Resuscitation Remember to contact EMS and attempt to get and AED and barrier devices
