Fitter grinding the pipe by using cutting disc Instead of grinding disc. Wooden pieces used as wedges scattered in the work location, noted as tripping hazard.

Rest shed used as the storage area at the pig receiving station.

Rest shed must be hygienically maintained for occupancy by the personnel on site. Provide storage area and label the areas as Rest Shed & Storage Area. Openings in the floor shall be barricaded and caution signs placed.

CNIA Camp 7 Noted openings in the floor as slip, trip and fall hazards. Closed JP ensure that work area should be clear from slip, trip, fall hazard and barricaded to prevent injury After the work tools return to store/ keep in the tool box
