The health card for boys and girls is a tool that integrates and systematizes
all preventive actions and facilitates the continuity of care; records the
compliance with the vaccination schedule and organizes regular check-ups from
birth to 5 years of age (and up to 6 years of age for
vaccination); it detects deviations from normal patterns early
of growth and development, used as an educational resource for the
mothers and fathers in family promotion and self-care.
In the Ministry of Health, the health card for boys and girls is an instrument of
record of the preventive actions provided in health services to
national level for comprehensive care of children under 5 years old.
It is used by mothers, fathers, and/or caregivers of children.

dental morbidity in children and adults. This form is used to obtain the
data for the monthly and quarterly production forms, said data is
