Understanding the Include Me Plan
The categories used in the Include Me plan correspond to those on the Foundations for Learning
framework. This framework emphasis language as the key component to effective learning and this is
reflected in our plans for all children.
The Include Me plan is written from your child’s perspective as they and their development are the
focus of this support plan. Each section provides information first on your child’s current level of
functioning “Where I am now”. This can include skills, preferences, things that your child is able to do
well and areas that may challenge them.
The second section, “Where I want to be at the end of this Include Me plan” is an outline of the goals
that we will be working towards during the period the plan is in place for. This is our end goal. It is
possible that your child may be able to achieve some goals before the end of the plan’s time frame.
Well done! If this happens we will look at extending their skills in this area and report this in our next
review session.
It is also possible that your child may not achieve some goals in the time frame set for this plan. This
can happen for many reasons, and that is not a problem. Some children just need more practice and
time to achieve a goal and your child’s progress is monitored and adjusted on an ongoing basis
throughout a plan. We will report the level of progress in our next review session and extend the goal.
We consider that your child has achieved a goal when they are able to independently (without
prompting) show the skill or behaviour 80% of the time, on three consecutive days or sessions, with
three or more different people, in two or more different settings. This is how we measure our goals.
Until your child can achieve this measure for a goal, we will keep working on it. As a big goal may be
made up of many different skills the goals in each plan may look different. We may break a big goal
down into smaller steps so that your child can learn the necessary skills and behaviours that they
need to achieve the end goal.

Initial sessions with Paul have been inconsistent due to family commitments over the Christmas
and New Year holiday season.
An informal skill and development evaluation was undertaken on the week beginning January 6 th,
2021. This support plan is based on the outcomes of this evaluation and observations taken
during the completed sessions.
