High hyperopia
8-year-old male patient, student in the fourth year of elementary school,attend to
I ask because her mother notices that she strains her gaze to try to read either in books or
on the computer, in addition to his level of concentration having decreased.
Family medical history:
Grandmothermother with diabetes
Fatherwith hyperopia
Motherwith astigmatism
Grandfatherfather with hypertension and cataract.
Personal medical history:
Conjunctivitisrecurrent since the age of 5.
Strabismusconvergent since the age of 2.
Surgical History:
Tonsillectomyat the age of 6 years.
Current Treatment:
Treatmentfor conjunctivitis, receive 2 ophthalmic drops of ciprofloxacin every 4
hours, he has been under treatment for 4 days.
Allergies to Medications:
He/She hasallergy to penicillin G, no allergy to another member has been confirmed
same group.
Toxic habits:
Feedinghigh in fats
Useexcessive devices more than 3 hours daily
Patient symptoms:

Ophthalmological examination
1. Examof the visual function:
- Visual acuity
Snellen scale:
Without glasses: 20/25
With converging lenses: 20/20
With pinhole: 20/20
- Vision colors
Ishiharathe patient was able to recognize the 18 plates, could not differentiate the
numbers in plates 18, 19, 20, 21, with normal color vision.
- Visual field by confrontation
Central visual field affected, due to difficulty seeing nearby objects.
2. Examof the structures of the visual system:
Attachmentsoculars:
Presence of hyperemic conjunctivae with scant secretions
mucopurulent.
Mild ocular hyperemianot irradiated.
Mobilityextrinsic ocular
Convergent strabismus +45°.
Segmentanterior of the eyeball
Presence of flat cornea.
Reflectionspupils:
Present consensual reflection.
Isocoric pupils
Reactivity
Tensionocular: soft eye (normal).
Mediarefringent
Cornea: transparent
Sclera and bulbar conjunctiva: normal
Intermediate lighting of the iris.
The three Purkinje images are observed.
Presence of the red-orange reflection
