comprising of best elements from google forms or Microsoft forms, survey sparrow and tier

Multi-Rater Leadership Perception Survey
This survey is designed to gather perception-based feedback on leadership behaviors. The insights
will be incorporated for the design of the leaders’ development journey. Please rate the leader on the
following behaviors using the scale provided.
All questions are mandatory for the participant to answer before being bale to submit

One participant may be submitting a survey in more than one capacity and for more than one leader
but we need to ensure the design in such a way that this participant should get a unique link and
should be able to take the survey anonymously without having to disclose his or her name e-
mail address or any such details.

Duplicaitons should be not be there

The first two questions are to be a drop-down selection. For question 2 admin should have the
option of adding the leader names directly from the admin module and also import via a cvs or excel
file.

3-18 onwards we are doing the actual survey on a Scale: 1 = Rarely | 2 = Sometimes | 3 = Often | 4 =
Consistently | 5 = Role Model. this should be a check box

19 – 21 are Text Questions


19. What specific strengths of this leader contribute most to business and team success? ( short
answer)
20. What specific behaviors or habits should this leader change to increase
effectiveness? ( paragraph)
21. Share any examples or stories that illustrate how this leader makes a difference. ( paragraph)

When they finish they get a thank you form and a reminder with names of the other people
they need to still submit their surveys for. So they way this will be designed os the admin
again has the right to assign who will be reviewing for whom and what capacity

On the reporting – again this should be similar to best practices of all google forms and
Microsoft forms with ability to benchmark participants score to group average across all
questions and come up with charts and graphs
