Opening
Student:
Good morning po, Ma’am/Sir. I’m John Dominic Pascua, a 4th year nursing student. Today, I will
be conducting an interview using the Training Needs Assessment Tool for Community Health
Nurses. The purpose of this interview is to understand your experiences and training needs as a
community health nurse. Your responses will be kept confidential and will only be used for
academic purposes. Is it okay po if we begin?
Part I. General Information
1. Can you please share some basic information about yourself?
o Name (optional po).
o Sex po (Male, Female, or Other).
o Age po.
o Current position or designation po (e.g., staff nurse, clinical instructor, community
health nurse).
o Ilang taon na po kayo sa community nursing practice?
o At saan po kayo naka-assign na barangay or community?
Part II. Competency Self-Assessment
Instructions to Professor/CI:
For the next part, I’ll ask about your competency in different areas of community health nursing.
Please rate yourself from 1 to 5, kung saan:
1 = No competence (wala pang knowledge/experience)
2 = Basic knowledge (may alam pero kailangan pa ng malawak na training)
3 = Moderate competence (may practice na pero needs improvement)
4 = Competent (nagagawa independently)
5 = Highly competent (mastery level, can teach/mentor others)
I will also ask kung sa tingin niyo po kailangan niyo ng additional training in that area.
Community Assessment & Planning
How competent are you in conducting community health needs assessments? (Pag-
identify ng health problems at resources ng isang barangay o community.)
How about household or family health profiling? (Pagtatala ng health information ng
bawat pamilya sa community.)
How competent are you in identifying social determinants of health? (Pag-analyze
kung paano factors like poverty, education, trabaho, o environment nakakaapekto sa
health.)
How do you rate yourself in program planning with community participation?
(Pakikipag-collaborate sa barangay officials, BHWs, at community members para
magplano ng health programs.)
Health Promotion & Disease Prevention
How skilled are you in health education & promotion strategies? (Pagbibigay ng
lectures, IEC materials, or health campaigns.)
How about immunization and cold chain management? (Pag-handle ng vaccines,
storage, at delivery.)
How competent are you in nutrition education & growth monitoring? (Pag-assess ng
nutrition status at pagtuturo ng tamang pagkain.)
How about mental health promotion? (Pagbibigay ng awareness, counseling, at
support related to mental health.)
Maternal, Child & Family Health
How competent are you in antenatal and postnatal care? (Pag-aalaga sa buntis at
postpartum mothers.)
How about safe motherhood practices? (Pag-educate sa mothers about safe
pregnancy and delivery.)
How skilled are you in growth and development monitoring? (Pagsusuri ng growth
milestones ng bata.)
How about adolescent and reproductive health? (Pagtuturo tungkol sa puberty,
reproductive health, at sexuality education.)
Communicable & Non-Communicable Diseases
How competent are you in TB, HIV/AIDS, and STI prevention and control? (Pag-
monitor, screening, and referral.)
How skilled are you in hypertension and diabetes management at community level?
(Pag-check ng BP, blood sugar, at patient education.)
How about lifestyle modification counseling? (Pagtuturo ng healthy diet, exercise, at
pag-iwas sa bisyo.)
Environmental & Occupational Health
How competent are you in sanitation and waste management? (Education sa tamang
waste segregation at disposal.)
How about water, hygiene, and vector control? (Safe water practices, handwashing
campaigns, dengue prevention.)
How skilled are you in workplace/community safety education? (Safety awareness
programs sa barangay or workplaces.)
Disaster Preparedness & Emergency Response
How competent are you in community disaster risk reduction and management?
(Pag-organize ng drills at disaster planning.)
How about first aid and basic life support? (Emergency response skills like CPR and
wound care.)
How skilled are you in outbreak response and surveillance? (Pag-monitor at pag-
handle ng outbreaks like dengue or measles.)
Leadership & Collaboration
How competent are you in coordinating with barangay health workers (BHWs)?
(Supervision at guidance sa kanila.)
How about intersectoral collaboration? (Working with LGUs, NGOs, and other
organizations.)
