Hello,

I am writing you to request office visit notes regarding our mutual patient Patient name or DOB We
received a physician written order but the patient recent office notes were not included along with the
signed order. To process this request further we require a copy of patient office visit notes of the last 6
months. Patient is under the care of DR Name and being treated in your office. We have to process the
request of our patient on urgent basis hence we require copy of patient office notes as it will expedite
the process and we can go ahead with the shipping.

I am attaching a copy of a written order that we received. Please send us office notes of the patient to
the fax number 786-842-7680 as soon as possible.
