I, __________________, of legal age, married, and a resident of ________________, do hereby authorize


my mother, _____________, of legal age, likewise a resident of ________________________________,
to act and transact on my behalf in relation to my Birth Certificate with the Philippine Statistics Authority
(PSA), the Local Civil Registrar (LCR) and/or other government agencies.

She is authorized to:


1. Request, process, and secure copies of my birth certificate;
2. Sign any necessary forms and documents related thereto;
3. Pay the require fees, receive official receipts, and accept and release documents on my
behalf and;
