This memo formally announces the implementation of our numeracy intervention program,
"Project NUMBERS: Step by Step to Math Success!," for struggling learners in Grades 3 and 4. The program
will be conducted from August 2025 to March 2026.

This initiative is a critical part of our commitment to addressing learning gaps and building a strong
mathematical foundation for our students. The sessions will be led by me, the dedicated Math teacher,
and will involve a variety of engaging activities to enhance multi-digit operations, fractions, and problem-
solving skills.

As the Teacher-in-Charge, I urge all stakeholders to support this initiative to ensure the success and
well-being of our students.

MEMORANDUM OF AGREEMENT
This Memorandum of Agreement (hereinafter referred to as "MOA") is made and entered into this 15th
day of August, 2025, by and among:
MR. BERNARDO B. CUBIT, of legal age, Filipino, and a Math Teacher/Teacher-In-Charge at Magtoma
Elementary School, hereinafter referred to as the "Intervention Teacher";
-AND-
The undersigned parents/guardians of struggling learners from Grade 3 and Grade 4, whose names and
signatures appear below, hereinafter collectively referred to as the "Parents."

WITNESSETH THAT:
WHEREAS, the Intervention Teacher is dedicated to providing additional support and guidance to students
who are experiencing academic difficulties in Mathematics;
WHEREAS, the Parents acknowledge the need for focused numeracy intervention to help their children
overcome learning challenges and improve their academic performance;
WHEREAS, both parties desire to collaborate effectively to ensure the success and well-being of the
struggling learners;
NOW, THEREFORE, for and in consideration of the foregoing premises and the mutual covenants
hereinafter set forth, the parties hereby agree as follows:

I. Purpose of the Agreement


This MOA outlines the roles and responsibilities of the Intervention Teacher and the Parents in
implementing a daily afternoon numeracy intervention program for struggling learners from Grade 3 and
Grade 4.
II. Scope of Intervention
The Intervention Teacher, Mr. Bernardo B. Cubit, agrees to:
1. Conduct every afternoon of Monday to Thursday intervention sessions for identified struggling
learners from Grade 3 and Grade 4. These sessions will be held from 11:30 to 12:30 PM on regular
school days.
2. Assess the specific learning needs of each struggling learner and tailor appropriate intervention
strategies for numeracy.
3. Provide individualized or small-group instruction focusing on multi-digit operations, fractions, and
problem- solving skills.
4. Monitor the progress of each learner and maintain records of their development.

5. Communicate regularly with Parents regarding their child's progress, challenges, and strategies for
home support.
6. Coordinate with the learners' regular classroom teachers to ensure alignment of intervention efforts
with the school curriculum.

III. Responsibilities of the Parents


The Parents agree to:
1. Ensure their child's consistent attendance in the daily afternoon intervention sessions.
2. Provide a conducive learning environment at home and support their child in completing any
assigned tasks or practice activities.
3. Actively communicate with the Intervention Teacher regarding their child's progress, concerns, or
any factors affecting their learning.
4. Attend scheduled parent-teacher conferences or meetings to discuss their child's development.
5. Collaborate with the Intervention Teacher in implementing recommended strategies for home-based
learning support.
6. Inform the Intervention Teacher in advance of any absences of their child from the intervention
sessions.

IV. Duration of the Agreement


This MOA shall commence on August 2025 and shall remain in full force and effect until the end of the
current academic year, March 2026, unless terminated earlier by mutual agreement of both parties or as
otherwise provided herein.
V. Confidentiality
All information regarding the struggling learners' academic performance and personal circumstances
shared during the intervention program shall be treated with utmost confidentiality by both the
Intervention Teacher and the Parents.
VI. Amendments
Any amendments or modifications to this MOA must be made in writing and mutually agreed upon by all
parties.

I. Background and Rationale A strong foundation in numeracy is a non-negotiable prerequisite for
academic success and real-world problem-solving. As learners progress to Grades 3 and 4, the
mathematical concepts become more abstract and complex, moving from basic operations to multi-digit
arithmetic, fractions, and more intricate word problems. Without targeted support, struggling learners can
develop significant learning gaps, leading to math anxiety and a decline in overall academic performance.
This "Project NUMBERS" is designed to provide a structured and supportive environment where Grade 3
and 4 learners can revisit foundational concepts and build the necessary skills to confidently tackle grade-
level mathematics.
II. Objectives The proposed "Project NUMBERS" Intervention Program was conceptualized with the
following specific objectives:
1. To enhance the multi-digit operation skills (addition, subtraction, multiplication, and division) of
struggling learners in Grades 3 and 4.
2. To improve the conceptual understanding of fractions and their basic operations.
3. To develop the ability of participating learners to analyze and solve multi-step word problems.
4. To cultivate a positive attitude towards mathematics and foster a growth mindset in learners.
III. Identification of Target Learners Struggling learners from Grade 3 and 4 will be identified through a
combination of:
1. Teacher observations and recommendations based on classroom performance.
2. Results from an initial diagnostic numeracy assessment (pre-test) that covers key Grade 3 and 4
competencies.
