With good contractions, patient was asked to bear down when patient was fully
dilated . Fhs baseline……….. Patient given lithotomy position , drapping and
painting was done. When crowing was present. Episiotomy given on left Medio lateral
.A single live ………..child delivered by vertex presentation on (Date) at (Time)
weight ……. with liquor…………. Pediatric call given at ………….. and pediatric call
attended at ……………. and by …………….. Baby cried immediately after birth and kept
between breast of mother .cord clamp and cut after 2-3 min. Cord sample sent/ not.
No any congenital anomaly detected. 3 orifices(nasal ,oral, anal) present in baby.
Placental and cord examination done – NAD.
AMTSL done .Placental weight ……..gm.
Pediatric instructed baby with mother on breat feeding/ shift to nicu.

P/A: Uterus well contracted.
P/V: No active bleeding. No tear / pph.

EPISIOTOMY
• Vaginal mucosa sutured with vicryl 1 by continuous locking manner.
• Simple interrupted suture of perineal muscle taken with vicryl1.
• Vertical mattress suture of skin taken with vicryl 1.
• Episiotomy suture taken by Dr……….
• Haemostasis achieved, no active bleeding.
• Check P/V done by Dr………..
