II. Basis of Inspection
Complaint-based Inspection

Private Establishment in Barangay Poblacion 4, General Luna, Surigao
del Norte
III. Scope of Inspection
The inspection covers compliance of the facility with relevant laws, rules, and
regulations, including but not limited to:
1. LGU Business Permits and Licensing Requirements
2. Department of Health (DOH) requirements for licensure of health facilities and
laboratories
3. Food and Drug Administration (FDA) requirements for licensing of drug
outlets/pharmacy and radiological services
4. Bureau of Internal Revenue (BIR) requirements re: issuance of official receipts
5. Other applicable national and local regulations.
IV. Documents Requested/Presented
• FDA License to Operate (Pharmacy)
• BIR Registration and Official Receipts
• Business Permit from LGU

V. Observations
• The clinic has been issued a valid business permit for the calendar year 2025 by
the BPLO and was verified by Engr. Espejon, but it was noted that the facility has
signed a Memorandum of Undertaking for some of the requirements.
• The facility is BIR compliant and issues official receipts, however their receipt book
is depleted. They are currently issuing cash invoice instead. In addition, the facility
is in the process of updating its BIR registration from non-VAT to VAT.
• The clinic has a license to operate a drugstore/pharmacy and is valid until June
15, 2026.
• The facility performs diagnostic laboratory and radiological procedures, which
aids in the diagnosis and management of patients, however is currently operating
without license.
