RESEARCH SURVEY QUESTIONNAIRE

This questionnaire is designed to assess the level of knowledge and perception on

the use of herbal medicine among residents of Barangay Lima, Pastrana, Leyte. Your

responses will remain confidential and used for academic purposes only. The

questionnaire consists of three sections: (1) demographic profile, (2) knowledge of herbal

medicine, and (3) perception of herbal medicine. Please answer all questions honestly.

Preparation: The leaves are
grinded and applied on the
skin.

2. Ampalaya
Preparation: The leaves may
be eaten as a salad or as an
ingredient in meals. It may
also be boiled with 2 glasses
of water until only half the
water is left.

3. Bawang
Preparation: The garlic bulbs
are soaked in vinegar, grilled
or fried in oil.

4. Bayabas
Preparation: The leaves are
boiled in water for cleaning
wounds or drinking.

5. Lagundi
Preparation: The leaves are
crushed and then boiled with
2 cups of water until only
half the water is left.

6. Niyog-niyogan
Preparation: The seeds are
taken out and eaten.

7. Pansit-Pansitan
Preparation: The leaves can
be eaten raw as a salad. The
leaves may also be boiled
with 2 glasses of water until
only half the water is left.

8. Sambong
Preparation: The leaves are
minced and boiled with 2
glasses of water until only
half the water is left. It is
then strained, and the
remaining water is for
drinking.

9. Tsaang Gubat
Preparation: The leaves are
chopped and boiled with 1
glass of water until only half
the water is left. It is then
strained, and the remaining
water is for drinking.

10. Yerba buena


Preparation: The leaves are
grinded and then boiled with
2 glasses of water until only
half the water is left. It is
then strained, and the
remaining water is for
drinking.
