2. REASON FOR CONSULTATION:

The patient is referred for neuropsychological evaluation due to presenting problems


to socialize and possible declining self-esteem.

3. CURRENT PROBLEM:

He/she shows shyness, at times remaining silent, averting gaze towards

4. BACKGROUND OF IMPORTANCE:

The evaluated individual consumed formula for a long period during their childhood.

6. OBSERVATIONS:

The evaluated person has a slender build, average height and weight for their age, complexion.

The evaluated individual shows an average IQ, and in some instances, they appeared somewhat
secure with the answers she expressed.

ATTENTION:

The evaluated individual presents a type of involuntary attention, because it depends on the
external stimuli, is at a level of selective attention because it selects
an object designated as the center of attention. On the other hand, it is at the level
II of consciousness, that is, (attentive vigilance).

PERCEPTION

Visual: Presents no alterations, performed each exercise with great calmness.

Auditory: Appropriately locates sounds, correctly discriminated rhythms.

Tactile: No difficulties were presented for the kinesthetic detection and tactile localization in
both hands. Moreover, it has adequate discrimination capacity, both in shapes,

MEMORY:

The evaluated individual presents adequate auditory and verbal memory. On the other hand, their memory
episodic memory is better preserved than its semantic memory. It is worth mentioning the
short-term memory is very inefficient due to a series of phrases and texts
read, which resulted in remembering less than half of the text.
However, it shows great ease in long-term memory.

THOUGHT:

The patient presents a concrete-functional type of thinking. In addition, the evaluated


demonstrates a suitable knowledge in reading and dictation of numbers; however,
presented difficulty in the performance of using calculation rules and operations
basic arithmetic for which it presents an arithmetic disorder and dyscalculia.
IMAGINATION:

The evaluated individual properly locates the parts of their body; thus giving as
resulting in a great ease for peripersonal and spatial recognition, furthermore,
demonstrated skills to recognize sequences of certain actions. Achievement
carry out creative work as expected and with great enthusiasm.

Conative Attitudes:

The evaluated person uses and applies all their abilities, there is consistency between what they say and
in the second one showed a higher degree of motivation, developed
the assigned tasks and collaborated with the examiners, She is a respectful girl,
