2.1 An investigator wants to study the effects of aniline dye on the development of Urinary bladder
Cancer. He identified the total number of Urinary bladder Cancer patients from local cancer registry.

2.2 A 7-year-old boy from an urban area in Indore is brought to a tertiary care hospital with high
fever (40.5°C) for 4 days. He also complains of intense headache, pain behind the eyes, severe body
and joint pains, and a reddish rash. The doctor notes the presence of petechiae. Over the last 24
hours, he has developed persistent vomiting and abdominal pain. His younger sibling has also
developed mild fever. The family lives in an area where water often stagnates in unused containers
and tires after rainfall. On investigation, the following findings were noted: NS1 Antigen: Positive,
Platelet count: 68,000/mm³, Hematocrit: 48%, Liver enzymes: Mildly raised
Based on the provided case, (2+6+5+7 Marks)
I. Identify the most likely diagnosis.
II. Describe its clinical course.
III. Explain its mode of transmission.
IV. Describe the role of key investigations in confirming the disease and assessing
its complications.
Q.3 Short Questions: - (5*6 = 30 Marks)
1. A well in a rural village has been contaminated with bacteria and viruses, posing a risk to the
health of the community that relies on it for drinking water. The well needs to be disinfected
to ensure the water is safe for consumption.
Outline the steps to disinfect the well.
2. 100 children fell ill with diarrhoea and vomiting after eating feast in a hostel event. What can
be the probable cause. What are the steps which you will take for investigation of the
outbreak.

3. Discuss the diagnostic algorithm of Paediatric Tuberculosis.

4. A 2-year-old child is brought to your Primary Health Centre (PHC) by his mother with
complaints of high-grade fever, dry cough, conjunctival redness, and generalized rash for the
past 3 days. On examination, the child is febrile (temperature 102°F), has a maculopapular
rash starting behind the ears and spreading to the trunk, and visible Koplik’s spots on the
buccal mucosa. The mother reports the child has not received any vaccinations since birth due
to family beliefs. Several similar cases have been reported recently in the community.

5. Rohit, a 2-month-old infant, is brought to the outpatient department with complaints of cough
and fever for the past 4 days and refusal to feed. The mother reports noticing fast breathing.
On examination, his respiratory rate is 64 breaths per minute, he appears irritable, and chest
indrawing is present. (2.5+2.5 Marks)
I. Based on the above presentation, outline the management plan for this child as
per IMNCI (Integrated Management of Neonatal and Childhood Illness)
guidelines.
II. Explain the IMNCI colour coding system, and specify which colour category
this child falls into and what it implies.

6. Sonam, a 25-year-old lactating mother, presents to the community health centre with
complaints of fatigue, weakness, and shortness of breath. She has been experiencing these
symptoms for the past few months. Sonam's dietary history reveals that she consumes a
predominantly vegetarian diet with limited intake of animal products. She has been
breastfeeding her 6-month-old baby exclusively.
