Chapter 3 – Standardized Tools

1. As a PSW you are a member of the interdisciplinary team. You observe, gather
information from the person and the family and, when needed and as appropriate,
you discuss with the tea, issues that affect the persons quality of life. You collaborate
to develop or update the care plan. Reflect on how you feel about collaborating with
the team. Write two (2) strategies you believe will help you to feel more confident in
collaborating with the team.

I feel good about team collaboration because it encourages creative problem-solving,


communication, and knowledge sharing. Two strategies that would help me feel more
confident when working with a team are:

1. Leading by example – showing respect and encouraging others to share their ideas.
2. Creating a positive environment – where everyone's input is valued and considered.

2. Caregivers in many countries and in many settings use the Palliative Performance
Scale (PPS) to identify the persons current level of functioning and needs, changes and
the need to adapt the care plan. What five things are measured on the scale?
i. Ambulation______________
ii. Activity and evidence of disease__
iii. Self care ______________
iv. Intake__________________
v. Consciousness level ________

3. Using words from the PPS, describe a person whose PPS is 20%

totally bed bound. Unable to do any activity and have an extensive disease. Needs a
total care, minimal to sips while feeding and might have full or drowsy confusion.

4. Using words from the PPS, describe a person whose PPS is 10%.
Unable to do any activity and have an extensive disease. Needs a total care and mouth care
only and may be in a coma status and no sign of conscience.

5. The PAINAD Scale is able to identify if the person is experiencing pain, but is not able
to identify the _____Intensity________ of pain.
