This Request is made on the basis that we accept relevant Rules and Guidelines of Asia Classification Society (further "ACS") and
"General condition of rendering of services by ACS".
We request that the following ship be surveyed by ACS for classification/transfer of class*. We agree to pay ACS's fees and the
expenses incurred by ACS for its services.

We hereby authorize your Society:
- to obtain from the losing/first Classification Society any information regarding the Status of Class, Surveys indicating overdue surveys,
outstanding conditions/recommendations for the subject ship;
- to demand copies of all survey records and relevant reporting of the losing/first Society for preparation of the ship history report and;
- the associated costs for copies will be charged to Owner or his representative;
Additionally we request to carry out the required Statutory Surveys and to issue Certificates on behalf of Flag State Authority.
