2.. Introduction Diabetes mellitus is a chronic
condition characterized by elevated blood
glucose levels, and it has become a significant
health concern worldwide. Traditional medicine
has explored various plant-based remedies for
managing blood sugar levels. The papaya plant
(Carica papaya) is known for its wide range of
health benefits, and its various parts, including
the stalk pith, are often used in folk medicine.
However, scientific data regarding the
hypoglycemic effects of papaya stalk pith extract
remains limited. This study aims to investigate
the hypoglycemic effects of papaya stalk pith
extract in diabetic animal models.

evaluate the potential hypoglycemic effects of
papaya stalk pith extract in diabetic models.
Specific objectives include: To assess the effects
of papaya stalk pith extract on fasting blood
glucose levels. To investigate the effects on
postprandial blood glucose levels. To examine
changes in insulin sensitivity and resistance. To
assess the biochemical markers of liver and
kidney function following treatment.
4. Hypothesis Papaya stalk pith extract lowers
blood glucose levels and improves insulin
sensitivity in diabetic animal models. 5
5.. Study Design 5.1 Type of Study This will be an
experimental, preclinical, randomized controlled
trial (RCT) conducted on diabetic rats. The study
will use both control and experimental groups to
determine the effects of papaya stalk pith
extract on blood glucose regulation. 5.2 Sample
Size A total of 30 male Wistar rats will be used,
randomly divided into 5 groups of 6 rats each.
5.3 Groups 1. Normal Control Group: Non-
diabetic rats treated with saline solution. 2.
Diabetic Control Group: Diabetic rats treated
with saline solution. 3. Papaya Stalk Pith Extract
Group (Low Dose): Diabetic rats treated with a
low dose of papaya stalk pith extract. 4. Papaya
Stalk Pith Extract Group (High Dose): Diabetic
rats treated with a high dose of papaya stalk pith
extract. 5. Standard Drug Group (Metformin):
Diabetic rats treated with standard anti-diabetic
drug (Metformin). 5.4 Animal Model Induction of
Diabetes: Diabetes will be induced in the rats by
administering a high dose of streptozotocin
(STZ) intraperitoneally (60 mg/kg). Rats with
fasting blood glucose levels above 250 mg/dL
will be considered diabetic. Papaya Stalk Pith
Extract Preparation: Fresh papaya stalk pith will
be harvested, dried, and then extracted using an
appropriate solvent (e.g., ethanol or water).
Extracts will be standardized based on their
active compounds (e.g., flavonoids, alkaloids). 6.
[Link] 6.1 Blood Glucose Monitoring Fasting
Blood Glucose: Blood glucose levels will be
measured before the treatment and after 7, 14,
and 28 days of treatment using a glucometer.
Postprandial Blood Glucose: Blood glucose will
be measured 1 hour after a standardized feeding
to assess postprandial effects. 6.2 Oral Glucose
Tolerance Test (OGTT) After 28 days of
treatment, an oral glucose tolerance test will be
conducted to assess glucose metabolism. Rats
will be administered 2 g/kg of glucose orally, and
blood glucose levels will be measured at 30, 60,
and 120 minutes post-glucose administration.
6.3 Insulin Sensitivity Insulin sensitivity will be
assessed using the homeostasis model
assessment of insulin resistance (HOMA-IR),
based on fasting insulin and glucose levels. 6.4
Biochemical Assays Liver Function: Alanine
transaminase (ALT), aspartate transaminase
(AST), and alkaline phosphatase (ALP) levels will
be measured. Kidney Function: Serum creatinine
and blood urea nitrogen (BUN) levels will be
evaluated. 6.5 Histopathology At the end of the
study, liver, pancreas, and kidney tissues will be
harvested, fixed, and stained for
histopathological examination to assess
structural changes due to diabetes and the
effects of papaya stalk pith extract. 7. Data
Analysis Statistical Analysis: Data will be
analyzed using one-way ANOVA followed by
post-hoc Tukey’s test to compare the differences
between groups. A p-value of <0.05 will be
considered statistically significant. Graphical
Representation: Graphs will be plotted for blood
glucose levels, insulin sensitivity, and
biochemical markers across different time
points. 8. Ethical Considerations All animal
procedures will be conducted in compliance with
institutional animal ethics guidelines. The study
will be approved by the Institutional Animal
Ethics Committee (IAEC). Humane treatment and
appropriate anesthesia will be provided to
minimize animal suffering. 9. Expected
Outcomes Primary Outcome: A significant
reduction in fasting and postprandial blood
glucose levels in the papaya stalk pith extract-
treated groups compared to the diabetic control
group.
