MASTER TEMPLATE: LEAN SIX SIGMMA PROJECT SELECTION

Dear Team,
We are pleased to provide you with a standard template designed to streamline the processes of stakeholder identification, project charter for your company's projects. This comprehensive template will foster a
structured and efficient approach to executing Lean Six Sigma methodologies across your projects.
1. Stakeholder Identification RACI Chart: This powerful section of the template delineates clear roles and responsibilities for all key stakeholders involved in the Lean Six Sigma initiatives using the RACI
(Responsible, Accountable, Consulted, Informed) framework. By mapping out the decision-making structure and communication flow, everyone will understand their specific roles and contributions to the
project’s success.
2. Project Identification and Financial Justification: This section assists in capturing and justifying potential Lean Six Sigma projects based on monetary benefits. It includes analyses of key financial indicators
such as the Internal Rate of Return (IRR) and Net Present Value (NPV) to justify project selections.
Utilization Instructions: Please meticulously fill in the data across all three sections for every single project idea. Attach tutorials and additional resources as well; these will guide you step-by-step through the process
of effectively utilizing the template. This approach will enable you to systematically solve challenges and make informed decisions.
We encourage each group to compose up to three high-quality ideas maximum. The standard template provided below is to be used for all your analyses.
It is our hope that this template will become an integral tool in your company’s continuous improvement efforts. Should you require further assistance or a more detailed walkthrough, please do not hesitate to reach out.

• Responsible: Who is responsible for doing the work?

• Accountable: Who makes the final decision and has ultimate ownership?
• Consulted: Who provides input based on their expertise?
• Informed: Who needs to be kept informed about the project's progress?

2. Project Charter
Subject: Essential Criteria for Project Selection
Dear Lean, Six Sigma Team,
To ensure the success of our Lean Six Sigma projects and the attainment of our operational excellence goals, it is imperative that each project we select adheres to the following criteria:
1. Specific Target: The project must have a well-defined objective, focusing on a specific area that requires improvement. Ambiguity in the project's targets can lead to misdirected efforts and suboptimal results.
2. Measurable Goals: Objectives should be quantifiable to track progress effectively. We must be able to measure the impact of our improvements with clear metrics.
3. Data Availability: Adequate data must be readily available for the existing process to establish a baseline and to measure against as improvements are made. This will facilitate a factual understanding of the
process performance pre- and post-intervention.
4. KPI Alignment: The Key Performance Indicators (KPIs) must be directly linked with the process to be improved. This linkage is crucial for monitoring the project's impact on operational performance.
5. Achievable Outcomes: The project goals must be realistic and attainable within the resources, time, and budget constraints of the organization.
6. Alignment with Strategic Objectives: Each project should align with the broader strategic goals of the organization. Projects that do not support these goals may not be the best use of our resources.
7. Stakeholder Impact: Consideration must be given to how the project will affect stakeholders. The selected projects should have a net positive impact on employees, customers, suppliers, and other relevant
parties.
8. Regulatory Compliance: The projects must comply with all relevant laws, regulations, and industry standards to avoid legal and financial penalties.
Adhering to these criteria will ensure that our Lean Six Sigma projects are not only well-defined and successful but also contribute to our strategic vision and operational excellence.

Proposed Project Charter# 1

Project Charter Description
Problem Statement Currently hourly production rate varies. In start of shift it remains low and at end of shift of it goes high. This production rate variation
(Give $ Justification) reduces the quality sugar percentage. Last year percentage of quality sugar is 33.92%. By reducing production rate variation will increase
this KPI from 33.92% to 34.10%. This increased quality sugar percentage will give benefit of 4025$.
Project Background Project Background:
In the context of continuous improvement and operational excellence (lean six sigma), this project is focused on enhancing the quality
percentage of our processes by reducing process variation. Over the past two years, Almoiz-II has experienced quality sugar percentages of
33.892% and 33.916%, which indicate a consistent but suboptimal level of performance. The primary challenge identified is the significant
process variation, which has been directly impacting the percentage of quality sugar.

The selected Key Performance Indicator (KPI) for this Lean Six Sigma project is the percentage of quality. Analysis has shown that the
current process variation is the main contributor to the lower-than-desired quality levels. By systematically identifying and reducing these
variations, aim is to increase the quality percentage, thereby improving overall product quality and customer satisfaction.
The goal of this project is to achieve a projected quality percentage of 34.100% in the upcoming year. This will be accomplished through a
detailed analysis of the factors contributing to process variation, followed by the implementation of targeted improvements to stabilize and
standardize the process. The success of this project will not only result in higher quality products but also contribute to the overall efficiency
and competitiveness of operations.

RISK LEVEL GUIDELINES FOR ACTION

HIGH • Immediate intervention from the top management is needed to ensure that a concrete controls is in place to avoid occurrence
[15~ 25
Points] • Control established must be able to reduce the Risk to at least Medium Level
• For 10 points and above, must be given with additional control to reduce the risk level
MEDIUM
• For below 10 points additional control may be taken if there are any opportunity to do so
[5~12 Points]
• Medium Risks shall be reviewed periodically to ensure that they are not turned into High Risk

• Not compulsory to have additional control but it may be done voluntary

LOW
• Shall be reviewed regularly to ensure that they are not turned into High Risk
[1~4 Points]
• Reduce the risk to as low as possible & practical

Comments by LSSBB Team Lead for project selection justification This project is zero investment project. For this project reasons of process variations will be addressed and staff will be trained to reduce
these variations and this will increase percentage of quality sugar.

Project Team Leader Declaration The Lean Six Sigma Black Belt (LSSBB) team affirms that they have confirmed the availability and completed a thorough evaluation of all
necessary data for the selected processes. This data-driven approach ensures that each project is grounded in reliable evidence and aligns
with the rigorous standards of Lean Six Sigma for effective decision-making and sustainable improvement. The team is committed to
transparency, accuracy, and upholding continuous improvement principles. Also I confirm that all data needed is available
