a. Memorandum from Chief of Police with blotter entry number 2025-22-


067, dated January 22, 2025, with some subject as above.

2. Physical Injury and Damage to Property incident transpired on January 22,


2025 at the this pertains to the Vehicular Accident Reckless Imprudence
resulting in Serious Physical Injury and Damage of property, Barangay San
Felipe, Camarines Sur, Naga City at about 08:05 in the morning, in front of
Caltex Gas Station.

3. On February 1, 2025 at about 3:00 a o’clock in the afternoon investigator on


Case PSSg Ramos conducted follow up investigation and request CCTV
footage to the near incident, IOC showed mugshot of request CCTV footage
of establishment near the area before the said incident.
