Injury

DATE : January 22, 2025

I. AUTHORITY:

1. Standard Operating Procedures of this Police Unit with reference to incident


recorded in Patrol Section Incident Logbook, Entry No. 2025-22-067, date January
22, 2025.

II. DETAIL:

2. Upon receipt of the report from the Chief of the Patrol Section, duty Desk
Officer PSSg Roland Ramos and PCpt Fernandez were dispatched to investigate the
motor vehicle traffic accident involving the motorcycle driven by Gilbert Atienza.

3. At the scene, the undersigned investigator observed the Toyota Vios was
going to Naga City and upon reaching the said place of the incident incautiously
bump to the approaching Honda Click.

4. Other details of the incident were the Toyota Vios wrongfully overtaken a
truck on a curve path road. As a result, the motorcycle and the car collided causing
damages to both vehicles and the drivers of the Toyota Vios sustain minor injuries
while Gilbert Atienza the driver of the motorcycle sustained fatal injury they were
brought to the Naga City Hospital for immediate treatment.
III. RECOMMENDATION:

5. It is respectfully recommended that the subjected Toyota Vios was brought


to the station for proper disposition.
