AUTHORITY
1. Inherent authority of the undersigned to investigate any incident related to the
breach of peace and order within the area of respossibility.
II. MATTERS TO BE INVESTIGATED
2. To determine the surrounding circumstances of the killing of the said Doctor.
3. To possibly identify the suspect that will lead to his apprehension and possible
prosecution.
III. FACTS OF THE CASE
4. At about 7:00am in the morning of June 16, 2025, a shooting incident transpired
at Calamba, Laguna, victimizing Jose Mercado Rizal, 34 years old, Doctor, resident
of Calamba, Laguna. Initial Investigation revealed that, Jose Rizal was going to
his Clinic when all of a sudden motorcycle arrived. Thereafter, the suspect Andres
Malvar fired upon the doctor hitting in different parts of his body.
5. The SOCO Team was immediately deployed to process the crime scene. Revovered
pieces of evidence were brought to Regional Forensic Unit for ballistic and
paraffin examination.
6. Operation Manhunt is being undertaken led by the undersigned for the possible
arrest of the suspect. Checkpoints were already installed to seal all possible
routes. Immediately after the incident elemends of this station led by PSSG Max
Alvaro, established a checkpoints operation. Consequently, a black Honda RS with a
plate number 922 BCE match the description of the vehicle used by the suspect
approached the said checkpoint of the along Maharlika Rd.
8. At this juncture, the personnel of this station immediately flagged down the
said vehicle which led to his instantenous arrest. The arrest suspect were
identified as Andres Malvar 30 years of age, unemployed, resident of Calamba,
Laguna.
9. Bookings, fingerprint, and mugshots were taken from the suspect. He were also
subjected to physical and medical examination.
10. At the crime scene, the responding officer called on the investigator-on case
(IOC) to assist in the situation. Upon arrival, the IOC realized the need to seek
the assistance of SOCO. He informes the Tactical Operation Center to contact SOCO
and to proceed at the scene. Minutes later, SOCO team started tonprocess and
document rhe scene to include the photographs, note taking, and sketching of the
crime scene.
IV. INVESTIGATION/FINDINGS
11. Suspect were subjected to paraffin test. Result of which attached here with
possible result for ammonium nitrite.
12. Request for ballistic and cross-matching of firearms used was also made.
Result of the firearm recovered from the suspect matched with the shell recovered
at the crime scene.
13. Death certificate issued from the Local Civil Registrar was already taken. This
is to prove the death of the Doctor.
V. CONCLUSION
14. It is strongly believed that the case prepared, supported by the pieces of
evidence recovered. Consequently, the case filed in could ultimately led to the
conviction of the suspect.
VI. RECOMMENDATION
15. In view of the foregoing, considering that the suspect was arrested and the
case were filed before the court, the undersigned respectfully recommend that this
Investigation Report be forwarded to the Office of the Regional Investigation and
Detective Management Division for reference and pursuant.
