1. Reference: Spot Report dated June 16, 2025.

2. This pertains to the shooting incident that transpired around 7:00 am on June
16, 2025, at Calamba, Laguna, wherein the victim was Jose Mercado Rizal, 34 years
of age, a doctor, and a resident of Calamba, Laguna. While the identified suspect
is Andres Malvar, 30 years of age, unemployed and also a resident of Calamba,
Laguna.

3. As to the motive of the suspect, the victim, Jose Rizal was having an affair
with the wife of Mr. Malvar. And out of anger, he went to Rizal's Clinic and killes
him.

4. The arrested suspect and the recovered pieces of evidence were later brought to
the Regional Forensic Unit for ballistic and paraffin examinatin. Subsequently, the
suspect was brought to Hospital for physical and medical examination prior to his
turnover to the custodial facility for proper disposition.

5. The cadaver of the victim was moved to the Crime Laboratory Office for autopsy
examination. Then taken to the morgue.
