MEMORANDUM OF AGREEMENT
On the Enrollment of Michael Libot under Modular Distance Learning Program
School Year 2025–2026
This Memorandum of Agreement (MOA) is entered into this ___ day of___________, 2025,
by and between:
San Isidro National High School, a public secondary school duly organized and existing
under the Department of Education, located at Brgy. San Isidro, Trento, Agusan del Sur,
represented herein by its School Head, Mr. Norberto O. Cullantes, Jr., hereinafter referred
to as the “School”;
and
Emily Abiao, of legal age, Filipino, residing at Purok 7, San Isidro, Trento, Agusan del Sur,
parent/guardian of Michael Libot, hereinafter referred to as the “Parent/Guardian.”
WHEREAS, the Parent/Guardian seeks to enroll her child, Michael Libot, for School Year
2025–2026 at San Isidro National High School;
WHEREAS, due to circumstances affecting the learner’s capacity to attend in-person
classes, the Parent/Guardian has requested that the learner be allowed to undergo the
Modular Distance Learning Modality;
WHEREAS, the School recognizes the right of every Filipino learner to quality and
accessible education regardless of learning circumstances;
WHEREAS, pursuant to Republic Act No. 9155, otherwise known as the Governance of
Basic Education Act of 2001, and DepEd Order No. 021, s. 2019 (Policy Guidelines on the
K to 12 Basic Education Program), schools are mandated to provide appropriate learning
delivery modalities based on learner needs and local context;
WHEREAS, DepEd Order No. 012, s. 2020 (Adoption of the Basic Education Learning
Continuity Plan for SY 2020–2021) institutionalizes distance learning options, including
modular learning, as valid delivery modes;
NOW, THEREFORE, for and in consideration of the foregoing premises, the parties hereby
agree as follows:
1. The School shall allow Michael Libot to enroll and participate in the Modular Distance
Learning for SY 2025–2026;

2. The Parent/Guardian agrees to:
2.1. Support and guide the learner at home in accomplishing the self-learning modules;
2.2. Ensure timely submission of accomplished modules;
2.3. Cooperate with teachers in the conduct of monitoring and feedback mechanisms;
3. The School shall:
3.1. Provide quality, complete, and developmentally appropriate self-learning modules;
3.2. Monitor learner progress and provide appropriate academic interventions as needed;
3.3. Conduct periodic consultations with the Parent/Guardian;
4. Both parties agree that this arrangement is subject to review every quarter and may be
modified based on the learner’s performance and recommendations of the School.
