II. During the Meeting:


• Refer to the information on the form
• View advisee’s academic profile on Zambeel (navigation doc) (Zambeel Portal)

III. After the Meeting:


• Electronically sign the form, or let students know they can CC you when they email the form to

• If there is anything important that you wish to flag based on your conversation with advisees,
please upload as a note on Zambeel (navigation)

KEY INSTRUCTIONS:

I. Before the Meeting:


• Take out 15 minutes to think through and fill in this form; print the filled form take a hard copy with you to
your meeting with your Faculty Advisor.

II. During the Meeting:


• Share the form with the faculty at the start of the meeting
• At the end of the meeting, request your advisor to sign this form after the meeting.

Note:
It is not appropriate to ask the faculty questions on administrative matters, enrollment approvals, pending
graduation requirements and university policies during these meetings, you need to email OUSA at
[Link]@[Link] for this information.

III. After the Meeting:


• IMPORTANT: You must email this completed form to OUSA at [Link]@[Link] with
your Faculty Advisor cc-ed to confirm that the mandatory meeting took place, and your attendance can be
confirmed.

• The subject of your email should be: “Filled self- reflection form & confirmation of advisor meeting”
