• At the end of the meeting, request your advisor to sign this form after the meeting.

It is not appropriate to ask the faculty questions on administrative matters, enrollment approvals, pending
graduation requirements and university policies during these meetings, you need to email OUSA at

III. After the Meeting:

your Faculty Advisor cc-ed to confirm that the mandatory meeting took place, and your attendance can be
